中國是一個有著五千年文化和歷史沉澱的泱泱大國，而美國是一個多民族的國家，只有200多年的歷史。中美之間的文化差異是巨大的，這是歷史地理條件以及傳統思想等多種因素影響而造成的。
中國的傳統文化是以儒家思想為核心，歷來主張尊卑有別，長幼有序，並受到等級觀念的制約。而美國文化主要承襲了基督教中的新教思想，等級觀念淡薄。他們更注重個人權益，追求人人平等，沒有輩份間過多的禮節。在《刮痧》許大同自己發達以後，將老父親接到美國照顧，這是典型的中國人的思維，雖然父親對新的生活不能適應，但內心還是非常幸福的，並且認為這樣自己可以好好地孝敬父親。
電影中的另一個情節是兒子丹尼斯和朋友的兒子打架，許大同為給朋友面子當眾體罰自己的孩子，並堅稱：「打是疼罵是愛」而且這種當眾體罰是對另一方的尊重。然而這種思想對於美國人是不可理解的，這也是後來因為刮痧而引起的爭論中美國人認為許大同有著虐待兒童傾向的一個論據。

《刮痧》所表現的中國人和美國人的差異，不僅在中美兩國的法律、道德標准等方面，而且觸及法理、觀念，甚至中醫理論等諸多議題。中美之間文化差異還表現在很多方面，例如美國人習慣用肢體語言擁抱、親吻等來表達人與人之間的友好和親近，當人們相互交流時，美國人認為互相注視著對方的眼腈是一件十分重要的事。然而在中國，我們是不善於這種肢體交流的，很多情況下我們還認為直視一個人的眼睛是不謙虛或不尊重對方的表現。這種體現在思維方式，風俗習慣，宗教信仰和非語言交際上的差異是中美乃至中西方在經濟文化政治方面有著沖突的一個不可忽略的重要原因。
時空上的差異造成了中美—東西方文化、觀念上的不同，進而表現為行為上的沖突。差異性從時間上來說，主要表現為國家的傳統與現代之間的矛盾；從空間上來說，由於各國在地域、歷史、政治、經濟等方面的原因造成了中西方的差異，表現為民族傳統精神背景、文化背景、哲學背景等方面。中、西兩種文明之間存在文化差異是可以理解的，也是不可避免，要消除這種文化隔閡，就必須要相互增進了解，只有這樣才有可能消除誤會，正如周總理所說我們要求同存異，相信「無論是怎樣的文化溝壑，只要相互交流就能達到諒解。」
中美文化有著差異的同時，也有著對文化趨同、文化融合的一種嚮往。我要保持文化鮮活性與多元性的同時，增進對彼此的了解，加強交流和溝通，希望在不久的將來，一個「大同」的世界真的會出現在我們面前！
